Health Reliance Health Services Inc. serves individuals: children, adolescents and adults with primary diagnosis of Developmental disability who may have secondary diagnosis of mental illness. Beginning age will be children aged 5 and above. Individuals have deficits in adaptive living skills and are not able to live independently; all require some level of supported living assistance. It is expected that some of the individuals might also have chronic medical problems, which may or may not be related with their Developmental disability or mental illness. At some point providing support in the in-home setting, the individuals will likely require medical monitoring and or medication management.

HEALTH RELIANCE CARE SERVICES, INC philosophy of service includes the biopsychosocial approach to assessment and service planning. This approach looks at each person’s unique physical, psychological and social needs and strengths in order to help define short and long term goals and objectives. Physical needs are addressed through a focus on the maintenance of basic health (i.e., sleep patterns, nutrition, exercise, personal hygiene) and facilitating individual access to appropriate healthcare providers.

Psychological needs are addressed through the provision of counselling and supportive interventions, incorporating awareness of developmental issues, educational interventions to enhance stress management, prevent relapse and increase overall coping skills and linkage access to professional psychiatric services and facilitations of family involvement, community linkages, as well as support of peer interactions. We hire professionals and paraprofessionals with long years of experience and expertise in providing therapeutic services to individuals with developmental disabilities and dually diagnosis.
